IMPORTANT GUIDELINES FOR YOUR CHECK-UP

1) All check-ups are conducted by prior appointments only.
2) To fix up an appointment,please call on Tel:022-64521855/9820980748.
3) Please report at billing counter at 9:00 AM
4) Do not take anything (not even tea) after dinner at the previous night.All regular medicines and water can be taken. Inform the consultant about the regular medicines.
5) Please bring stool sample in a pathology the clean,dry plastic/glass container(the containers can also be collected from pathology laboratory a day before the check-up).
6) All check-ups are conducted subject to full advance payment.
7) Please bring your earlier medical records when you come for check-up.
8) In case any specific tests are not included in the scheme,but which are found to be necessary,these would be charged separately.
9) Please inform the lab technician if you are Diabetic. Ladies please inform the x-ray technician if you are pregnant and do not give urine for examination during menses.
10) Avoid heavy meal and alcohol 24 hours prior to the Health Check-up.
11) You will be examined by a reputed doctor.
12) Review consultation by Physician on the same Day with the reports.
